| A medical plan with prescription drug coverage and no
deductible. With Individual Care Blue, you are covered for many preventive care
services including health maintenance exams, gynecological exams, routine Pap
smears, laboratory and radiology services, well-baby care and immunizations.
Individual Care Blue is available to individuals and family members under age
65. |
| Value Blue PPO and Traditional |
| Lets you cover your family while keeping your monthly
payments low through an annual deductible. Value Blue includes the Affinity Rx
program, which lets you purchase prescription drugs at our discounted rate. |
| Young Adult Blue PPO and Traditional |
| Designed for individuals ages 19 to 30. Young Adult Blue
costs less than $48 a month. It offers both PPO and Blue Traditional plans and
includes the Affinity Rx discount prescription drug program. |
